Use of your genetic assessment’ raw data for analysis of certain health aspects

I have been well informed on the need and purpose of sharing raw data for genetic report generation. And I understand that such an act is purely health-oriented, and hence I have no objection in providing this raw data. Moreover I have been clearly informed on the privacy and confidentiality aspects of my raw data analysis; hence I give my informed consent. 

I am thoroughly explained, in my preferred language, on the following-

• The possible outcomes of analyzing the raw data from genetic assessment– both positive and negative
• Assurance that my raw data of genetic assessment will be used relevantly for assessing specific health conditions, and it shall be used anonymously as a health record/ research registry with its privacy maintained. 
• Purpose of generating a report from the raw data of genetic assessment is to suggest feasible lifestyle changes, based on predispositions, for good health sustenance and disease prevention in the long run
• Clarity on the fact that this assessment tells us the predisposition/ likelihood for certain health outcomes based on genes. It is neither designed to diagnose health conditions/ diseases, nor is it intended to be a medical advice
